Provides a range of support options designed to help people with ongoing health conditions to remain in their homes and to be as independent and safe as possible. Services include grooming and hygiene, bath assistance, mobility/lift assistance, medication assistance, nutritional assistance and family/caregiver respite. Eligibility and degree of service subsidy is determined by case manager.more info
